Okay. We admit it. This isn't the most interesting part of our web site.
However, we are providing the forms on this page in an effort to help
get you into the water as fast as possible once you get to Tobermory.
If you have navigated to this page to download forms, we have probably asked you to do so and you know what you are looking for. However, to review......... Before boarding any of our boats, we require a signed liability waiver from every diver and passenger. If you are the leader of a group that has booked a full boat charter, we can get you into the water faster if you download the "Divers Den Liability Waiver" and have every member of your group sign a copy of the waiver before arriving in Tobermory. Before participating in diver training activities, students must complete a Medical Questionnaire. The purpose of this questionnaire is to find out if you should be examined by your doctor before participating in training (and before arriving in Tobermory). The PADI form also includes a Physician signoff and instructions for the Physician (if necessary). The SDI Physician signoff is a separate form and is posted below.
